Istotą PCC jest respektowanie wartości, doświadczeń, potrzeb i preferencji pacjenta w planowaniu, koordynowaniu i świadczeniu opieki. Centralnym elementem tego modelu jest relacja terapeutyczna między pacjentem a osobami świadczącymi opiekę zdrowotną. Liczne badania wykazały, że wdrożenie PCC do praktyki przyczynia się do poprawy wyników opieki, lepszego wykorzystania zasobów, obniżenia kosztów i zwiększenia zadowolenia pacjentów z opieki.
